Also ask the provider to revoke the grant. Default true.

Defaulting to revoke is the fail-closed choice: "disconnect" means the app should no longer be able to read the user's files, and a row deleted from our database while the grant lives on at the provider does not achieve that — it only makes the remaining access invisible to us.
