Boots an app with the impersonation enricher prepended.
Pass adapter to drive requests through Express or Hono instead of the default Fastify inject — the same suite then runs unchanged on any adapter (the neutral 'http:enrichers'/'http:guards' buckets make impersonation and guards behave identically).
